Appointment Information

  FOR INITIAL APPOINTMENTS

Please bring with you:

• Your referral letter
  
• Your Medicare Card
  
• Details of current medications
  
• MRI and X-Ray results, if applicable
  
• Hearing test results, if applicable
  
• Any other test results relating to your ears, hearing or balance, from other clinics

To save time you may print and fill out a registration form before your appointment.

Important information to prepare for your appointment:

• Please arrive 10 minutes early to fill out paperwork
  
• Please don't wear any eye makeup
  
• Please refrain from eating or drinking at least one hour before your appointment
  
• Your appointment will last between 45 - 60 minutes

  FOR FOLLOW UP APPOINTMENTS

Please bring:

• Any relevant X-Rays, audiograms etc
  
• Results of new tests conducted by your general practitioner

  CANCELLING / RESCHEDULING APPOINTMENTS

If you need to cancel or reschedule an appointment, please do this as early as possible to ensure availability for another appointment or to allow the appointment time to given to another patient.
